For over two decades, the valuation and operational viability of health information technologies were dictated by classic software-as-a-service (SaaS) unit economics. These legacy frameworks relied on highly predictable variables, primarily measured through customer lifetime value (LTV), customer acquisition cost (CAC) and stable, subscription-based licensing models. &nbsp;&nbsp; However, the emergence of non-deterministic, generative medical architectures has broken these traditional economic frameworks.&nbsp; The industry is rapidly transitioning toward "Unit Tokenomics", the practice of modelling, tracking and optimising the cost, consumption and business value of computational tokens as the fundamental unit of clinical intelligence and enterprise value.&nbsp; Within this new paradigm, tokens are not speculative cryptographic assets; instead, they represent the atomic unit of computation, discrete sub-word fragments of clinical text, pixel blocks of radiological images, or frequency bins of physiological audio. Länsi-, Keski- ja Itä-Uudenmaan hyvinvointialueet ovat luopuneet Apotista. Kerava-Vantaa ja Helsinki ovat tilaamassa konsulttifirma BearingPointilta selvitystä siitä, millaiset potilas- ja asiakastietojärjestelmät palvelisivat niitä parhaiten lähitulevaisuudessa. Yksi selvityksen kohde on mahdollinen Apotista luopuminen.Apotti on ollut käyttöönoton alusta alkaen julkisen ja ammatillisen arvostelun riepoteltavana. Tietojärjestelmää käyttävät ammattilaiset ovat kertoneet omilla keskustelufoorumeillaan ja julkisuudessa kokemuksiaan siitä, kuinka järjestelmä sotkee ja hidastaa asiakastyötä eikä lainkaan ”ymmärrä”, miten julkinen terveydenhoito ja potilas- ja asiakastyön rutiinit toimivat Suomessa.Yhdysvaltalaisyritykset eivät kyenneet vastaamaan suomalaisodotuksiin Apotin käytettävyyden ja käyttäjäkokemuksen surkeus on saanut tukea myös selvityksistä ja tutkimuksista. Tämän lisäksi julkiseen keskusteluun ovat nousseet &nbsp;terveydenhoidon ammattilaisten valitukset siitä, että Apotin huono toiminta vaarantaa potilasturvallisuuden. Tämän huolen tiimoilta yli 600 lääkäriä teki tietojärjestelmästä virallisen kantelun valvontaviranomainen Valviralle vuonna 2022. Vastauksena Valvira (nykyään Lupa- ja valvontavirasto) totesi Apotin pystyvän käsittelemään asiakas- ja potilastietoja ”lakien ja säännösten edellyttämällä tavalla”.Epic ja Cerner (nyk. Oracle Health) – yhdysvaltalaiset suuryritykset, jotka IT-alan johtava konsulttiyritys Gartner on nostanut EHR-taikaneliönsä leader-ruutuun – eivät onnistuneet täyttämään suomalaisten sairaanhoitopiirien (nykyisin hyvinvointialueiden) kunnianhimoisia toiveita ja suunnitelmia alueellisen terveyden- ja sosiaalihuollon integroiduista potilas- ja asiakastietojärjestelmistä. Ajatuksena oli, että ne ohjaisivat data- ja algoritmisesti sote-organisaation toimintoja sujuvammiksi, lisäisivät kustannustehokkuutta sekä tukisivat tiedolla johtamista ja datan toisiokäyttöä.Aster ei koskaan toteutunut ja Apotista tuli yksi luku Epic Systemsin surullisen kuuluisaan pohjoismaiseen saagaan. Sen ‘vehkeet’ ovat saaneet terveydenhuollon ammattilaiset takajaloilleen paitsi Tanskassa ja Suomessa myös Norjassa. Epic failure on niin tuttu käsite maailmalla, että esimerkiksi Nelson Advisors -konsulttifirman Ison-Britannian julkiselle terveydenhuoltojärjestelmälle (NHS) tekemä selvitys vuodelta 2025 nostaa Tanskan Sundhedsplatformin varoittavaksi esimerkiksi. Selvityksessä tiivistettiin, miten laajamittainen uudistus ’yhdellä rysäyksellä’ kärsi pitkittyneestä tuottavuuden laskusta, puutteellisesta testauksesta ja koulutuksesta, datan laatuongelmista sekä merkittävästä käyttäjätyytymättömydestä.Asterin ja Apotin jälkeen markkinatilanne muuttui Apotti- ja Aster-hankkeiden lässähtäessä markkinoiden pelikortit jaettiin uudestaan. Kansainväliset jättifirmat menettivät osuuttaan, kun Cerner poistui ja Epicin siivu kaventui. Kotimaistaustaiset yhtiöt valtasivat Suomen markkinat.Uusi jako oli paluuta suurhankkeita edeltävään tilanteeseen. Apotin piirissä on toki edelleen miljoonia ihmisiä, koska se hoitaa Helsingin ja Kerava-Vantaan ohella erikoissairaanhoitoa Uudellamaalla ja Meilahden sairaalakeskittymässä. Muutoin hyvinvointialueittaisen kattavuuden ja väestöpohjan mukaan laskettuna TietoEvry:n Lifecare ja CGI-Suomen Omni 360 ovat terveydenhuollossa hallitsevimpia toimijoita. Pohjois-Pohjanmaa ja Lappi tosin valitsivat ESKOn, joka pohjautuu paikallisesti kehitettyyn sairaalatietojärjestelmään.Ennen suurhankkeiden alkamista 2010-luvun puolivälissä Tiedon Effica ja CGI:n Pegasos olivat ylivoimaisesti yleisimmät perusterveydenhuollon potilastietojärjestelmät. Sairaaloissa oli käytössä näiden ohella myös CGI:n Uranus sekä ESKO. 2020-luvun puolivälissä sosiaalipalvelujen asiakastietojärjestelmien osalta TietoEvryn (Lifecare) ja Epicin (Apotti) rinnalle on noussut suomalaisyhtiö Mediconsult, jonka Saga-järjestelmä on hankittu Pirkanmaalle, Pohjois-Savoon ja pohjoiseen Suomeen.Valtaosa näistä potilas- ja asiakastietojärjestelmistä ei vielä ole käytössä hyvinvointialueilla. Jos hyvinvointialueiden digi- ja tietohallintojohtajien Lääkärilehden vuoden 2025 kyselyssä esittämiin arvioihin ja odotuksiin on uskomista, terveydenhuollossa tietojärjestelmäuudistus saadaan toteutettua vuoteen 2028 mennessä.Sosiaalipalvelujen osalta monilla alueilla asiakastietojärjestelmien hankinta on kuitenkin vielä kesken ja useilla alueilla järjestelmätoimittajien sovellusten muokkaaminen käyttökuntoon ja käyttöönoton valmistelu ovat vasta käynnissä. Vuosikausia tästä eteenpäin alueellista sote-tietohallintaa hoidettaneen välimalliin ratkaisuilla. Sitten kun pitäisi olla valmista, saattaa olla aika alkaa hankkia taas ”uuden sukupolven” välineitä.&nbsp;Potilastietojärjestelmän kehityskaari on ollut kuin kalliin kehän kiertämistä Vuosikymmenen puurtamisen jälkeen vallitsee Suomen potilastietojärjestelmien kehittämisessä paitsi välitila myös déjà vu -tunne. Sosiaali- ja terveydenhuollon tiedonhallinnan ja tietojärjestelmien yhtenäisyyttä tavoitellaan yhä eikä terveyden-ja sosiaalihuollon potilas- ja asiakasdatan integrointi ole juurikaan edistynyt. Myös sote-henkilöstön keskuudessa on edelleen suurta huolta, tyytymättömyyttä ja kritiikkiä potilas- ja asiakastietojärjestelmien käytettävyyttä kohtaan.Suomen huomattavimmassa väestökeskittymässä eli valtaosassa Uuttamaata tietohallinto on jakautunut vanhaan tapaan eli perusterveydenhuolto käyttää eri järjestelmää kuin erikoissairaanhoito. Lisäksi potilas- ja asiakastietojärjestelmiä toimittavat hyvinvointialueille pääasiassa samat kotimaiset yritykset kuin ennenkin.On hankala sanoa, mitä tämän kehän kulkeminen on saanut aikaan. Yksi asia on kuitenkin selvä. Suurista sote-tietojärjestelmähankkeista Apotti sai järjestelmän käyttöön, sen tähänastiset hankinta- ja käyttökustannukset ovat noin miljardi euroa, ja keväällä 2022 TiVi-lehti raportoi Apotin käytön aiheuttavan 50 miljoonan euron lisäkustannukset vuosittain.Sikäli kuin Apotti-hanke ilmentää terveydenhuollon IT-kehittämisen ja dataistamisen eetosta, hankkeiden oikeuttaminen ”säästöillä” tuskin lienee kovin uskottavaa.&nbsp;Kirjallisuutta ”Apotti nosti Husin it-kuluja pysyvästi yli 50 miljoonalla vuodessa – ’Kuulostaa aika oudolta’”. It's the dominant gathering of the week, with company announcements (product launches, partnerships, research) being refreshed daily on the show floor and the Health Transformation Summit convening 200+ payer/provider CEOs and policymakers.FundingOn funding, the standout was Semble's £30M Series C, led by Revaia with Partech and Octopus Ventures, to expand its open, interoperable clinical platform into France and larger European groups. Around it, a cluster of smaller AI-led rounds: 01Health ($15M Series A, specialist healthcare infrastructure, UK),&nbsp;Uncovr ($7M for surgical AI handling post-op documentation and workflow), OurMind (€2.1M, Dutch, clinical admin automation), TurnUp (€2M, Ghent, reducing no-shows for medical/dental practices), and Nanordica Medical (€1.6M, Estonia, antibiotic-free chronic wound care).&nbsp;On the capital-formation side, Thena Capital closed a £45M debut fund, led entirely by female GPs, to back up to 25 early-stage health and MedTech startups. Tech.eu data shows capital concentrating into larger, commercial rounds, led by the UK (€2.5Bn), Switzerland (~€1.0Bn) and Finland (€881M).RegulationRegulation produced the most friction.
